Instructions To Complete COVID-19 Screening

  1. Click on the dropdown menu below and select the provider you are seeing at the office. This will be bring you to the COVID-19 Screening

  2. IMPORTANT- PLEASE READ THIS PRIOR TO CLICKING ON THE LINK BELOW: After completing the COVID-19 Screening and entering your e-signature there is still an additional step needed to complete the e-signing authentication: A pop-up will appear on your screen that will prompt you to enter your email address. Please make sure you enter an email address that you have access to on your mobile device. After entering your email address, an email will be sent to that address that you will need to open and click on the link in the body of that email. This last step will authenticate your e-signature.
